2. Consent and Release:

I, the undersigned, hereby grant SCARF Organization Inc. and its affiliates, representatives, and employees the right to use, reproduce, and distribute photographs, videos, and written success stories of me. This consent extends to use in any and all forms of media including, but not limited to, websites, social media, brochures, newsletters, and other promotional materials.

3. Specific Details:

Photos/Videos: I consent to SCARF Organization Inc. using photographs or video footage of me.

Success Stories: I consent to SCARF Organization Inc. using written success stories, testimonials, or quotes from me.

Purpose: The use of such materials is intended for promotional purposes, including but not limited to, showcasing the impact of SCARF Organization Inc.’s programs and initiatives.

4. Release of Liability:

I understand and agree that SCARF Organization Inc. shall not be liable for any claims, demands, or causes of action that might arise out of the use of the photographs, videos, or written success stories. This release includes any claims for invasion of privacy or defamation.

5. Duration:

This consent is given in perpetuity unless revoked in writing. I understand that my images and stories may remain on SCARF Organization Inc.’s materials even if I later withdraw consent.

6. Revocation of Consent:

If I wish to withdraw consent at any time in the future, I will provide written notice to SCARF Organization Inc. via email at mentoring@scarfinc.org or by mail at 1220 Patrick Street, Kissimmee, FL 34741. SCARF Organization Inc. will make reasonable efforts to cease further use of my images and stories upon receiving such notice.

I am signing this release form voluntarily. I am aware that this consent will remain in effect until a written revocation is received by SCARF Organization Inc.
